Interoception and the Insula in DPDR: Why the Sense of Self “Fades”
People with DPDR often describe it as “I have no body” or “I am not here.” Modern neuroscience links this to the insula — a key hub that integrates bodily signals (heartbeat, breathing, muscle tone) into the experience of “I am present.” In DPDR, the brain shifts into an “energy-saving” mode, reducing interoceptive input and relying more on cognitive control.
What happens in the brain
- Interoception is dampened. Under stress, the brain downregulates bodily signal processing to reduce emotional load (fMRI evidence: Critchley et al., 2004).
- Insula and limbic responses weaken, PFC over-activates. This imbalance — less insula/amygdala activity, more prefrontal control — was described in Sierra’s neurobiological model (2011).
- Subjectively: “absence of self.” With reduced bodily grounding, presence feels weakened, matching your idea of a fixation “node.”
Why this matters
- It is explainable and reversible: this is not damage but an adaptive shift, supported by recent predictive coding studies (Ludwig et al., 2024).
- Body-based practices — mindful breathing, grounding, movement — gradually restore interoception. Clinical trials (CBT, ACT) show benefit (Boyd et al., 2025).

DPDR is not the “loss of personality,” but a temporary disruption in how the insula integrates bodily signals. Restoring body tone through simple practices helps rebuild the sense of self — confirmed by neuroscience.
